TO MAKE AN APPOINTMENT IN CHARDON (currently online except for special cases):

• Choose your date and time on the online schedule and submit the request. You will get an email confirming your appointment within 1-2 days.
• New Patients: Please go to the Questionnaire tab inside your account to fill out the new patient forms.  This will take some time, but it will allow you to get more out of your visits if this is done before the appointment.
